When I saw the page I had filled it out and quickly emailed my sister the link on Monday, while they still had the form up for filling out. I think the 404 message was because it was due to high traffic, but today the promotion expired (at least according to the message they have up), but it seems more like they ran out of free samples of the gum to give away.
Looks like the promo ended. :-(
